Provide a direct Social Work service (compatible with the Hospital’s function) to the patients and/or their relatives as assigned to him/her at the discretion of the Principal Social Worker. To provide supportive counselling to patients and families while assessing their presenting needs, their community and social supports and any possible risks at hand.To ensure social work practice is in line with the guiding principles of the Assisted Decision Making (Capacity) Act 2022. Complete timely psychosocial assessment of patients and execute a formulated care plan with the multidisciplinary team. To fulfil all statutory requirements in relation to adult safeguarding, child protection, welfare and domestic violence.To work with patient/family to implement the Care Plan in an effective and timely manner. To actively engage in multidisciplinary team working at a hospital and community level as needed, in order to implement comprehensive care plans for patients. Keep and maintain such written and electronic records (including statistics) as are part of the structure/reporting of the Social Work Department.Carry out home visits where appropriate and in agreement with Principal Medical Social Work. To advocate for patients experiencing homelessness, referring to the appropriate agencies. Liaise with community health services both statutory and voluntary who are also concerned with the care and management of our patients.Participate in education/ training and development programmes for the Social Work service as arranged by the Department for example- Presentations in the area of Social Work, education, Bereavement Programme. Participate in any relevant research assigned by the Department Head. Partake in regular clinical supervision with a Senior Social Worker as designated by the Principal.Evaluate one’s practice in accordance with evidence-based standards and to initiate improvements. To ensure the necessary liaison and co-operation with Medical, Nursing and other disciplines in the Hospital so that the potential of the patient is maximised. To arrange for patient family meetings/Multi-Disciplinary Meetings in appropriate cases and include the relevant professionals in PCCC.To be available for supervision with a designated Senior Social Worker. Carry out any other duties appropriate to the office as may be assigned by the Principal Social Worker and the Hospital Management.
