About the Role
The Clinical Nurse Manager 1 (CNM1) plays a pivotal leadership role within the Primary Care Team, supporting the delivery of high-quality, accessible healthcare to individuals experiencing homelessness and other marginalised groups. The CNM1 will provide day-to-day clinical and operational leadership to the nursing team and Health Care Assistants across DSC Housing Services. The role will support the safe and effective delivery of onsite primary care services, including preventative care, chronic disease management, complex case coordination, and access to specialist services.Working closely with the Primary Care Nursing Team, multidisciplinary colleagues, housing teams, and external healthcare partners, the CNM1 will ensure strong clinical governance, staff support, and continuity of care for clients with complex health and social needs. The role also contributes to service development, workforce support, and strengthening partnerships across the homeless health network.Contract Type: Full-time, Permanent Location: Dublin Simon Primary Care Nursing team, working across multiple locations in Dublin City Centre. Team base: Ushers Island. Working Hours: 37.5 hours per week, 5-day liability Responsibilities
Support the CNM2 in the leadership, delivery, and ongoing development of primary care services within homelessness and community settings Provide clinical leadership and deliver high-quality, person-centred nursing care across homeless accommodation services Undertake comprehensive assessments, develop care plans, and ensure timely documentation and follow-up of complex health needsCoordinate care for clients with multifaceted health and social needs, acting as a key liaison between healthcare providers, social care, and housing services Advocate for equitable access to healthcare and improved outcomes for individuals experiencing homelessness Build and sustain positive working relationships with external healthcare providers, community services, and key stakeholdersAttend and contribute to Inclusion Health multidisciplinary team meetings Provide supervision, guidance, and support to nursing staff and Health Care Assistants Support team functioning within a multidisciplinary model, including specialist roles (e.g. Women’s Health, Tissue Viability) Work closely with housing providers to deliver integrated care and reduce barriers to service accessPromote preventative healthcare, early intervention, and engagement with primary care services Support and deliver on-site screening and health promotion initiatives Facilitate student nurse placements in collaboration with preceptors and academic partners Contribute to clinical governance, quality improvement, audit, and reporting requirementsWork flexibly across multiple sites to meet service needs and ensure continuity of care Qualifications and Experience
Registered General Nurse with active registration with the Nursing and Midwifery Board of Ireland (NMBI) Minimum 3 years post-registration nursing experience Experience working in primary care, community nursing, or outreach services Experience working with marginalised or vulnerable populations, including people experiencing homelessness, addiction, or complex social needsDemonstrated clinical leadership or management experience (e.g., acting CNM, team lead, service coordination) Experience managing complex clinical cases and coordinating care across services Strong knowledge of health inequalities and barriers to healthcare access Excellent communication and multidisciplinary collaboration skillsPassionate about advancing equity in healthcare services Previous experience in a CNM or supervisory role Experience working within homeless health services Experience working with people in addiction Further education in addiction, women’s health, chronic disease management, tissue viability, mental health, addiction, public health or end of life careAwareness of trauma informed practice Dublin Simon Community is an equal opportunities employer and encourage applications from all qualified individuals.
