Description
Occupational Therapist– Community Mental Health The Occupational Therapist will work as part of a multidisciplinary Community Mental Health Team (CMHT) to support individuals experiencing mental health difficulties in achieving optimal independence, recovery, and participation in meaningful daily activities. The rolefocuses on recovery-oriented practice within the community setting across County Louth.

Responsibilities

Provide occupational therapy assessment and intervention for adults experiencing mental health difficulties.

Assess functional ability, daily living skills, cognitive functioning, and environmental factors.

Develop and implement individualised recovery-focused care plans in collaboration with service users and the multidisciplinary team.

Deliver interventions such as:

Activities of daily living (ADL) rehabilitation

Vocational and educational support

Social inclusion and community participation

Anxiety and stress management strategies

Cognitive and functional rehabilitation

Facilitate group programmes promoting recovery, wellbeing, and skill development.

Support service users transitioning between hospital and community services where relevant.

Work collaboratively with psychiatrists, psychologists, nurses, social workers, and other allied health professionals.

Participate in case conferences, care planning meetings, and clinical reviews.

Contribute to risk assessment and management planning.

Conduct home visits and community-based interventions across County Louth.

Liaise with primary care services, housing services, employment and education supports, voluntary and community organisations.

Support service users to access community resources and supports.

Maintain accurate and timely clinical records in line with organisational policies.

Adhere to professional standards, policies, and procedures.

Participate in clinical audit, service development, and quality improvement initiatives.

Requirements

BSc (Hons) or equivalent qualification in Occupational Therapy.

Registration with CORU.

Experience working in mental health services (placement or post-qualification).

Knowledge of recovery-oriented practice and community-based mental health care.

Ability to work autonomously within a multidisciplinary team.
