Key Responsibilities To provide a nutrition and dietetic service to patients and their families/carers specifically in the area of diabetes and endocrinology. To deliver care within the multidisciplinary team approach. This includes multidisciplinary education and monitoring of clients within our care. To facilitate this approach, attendance at ward rounds, team and clinical review meetings is essential. To provide a dietetic service to Diabetic outpatient clinics and Day Care service as required. This may be virtual or face-to-face with patients and family members To provide a dietetic service to specialist clinics e.g. type 1 DM clinics, young person's clinics, preconception advice, pancreatic cancer and transplant and CF related diabetes. To ensure the patient care pathway for service delivery with community colleagues is optimised e.g. Model of Integrated Care for patients with Type 2 Diabetes ICGP and delivery of group education to patients with Type 2 diabetes. To provide dietetic cover as required for other members of the department during periods of leave. To participate in quality improvements in delivery of care through quality assurance projects, clinical audit or research. To initiate and participate in the education/training of dietetic colleagues, other health professionals, and patient groups as required. To design, prepare, and evaluate educational resources used for both patient and professional education. To facilitate the practice placement of pre-registration dietitians. Undertake co-ordination of practice placement education in collaboration with the other practice placement education co-ordinator and all members of the Dept., as required To maintain a portfolio of \"continuous professional development\" in the relevant areas of specialisation. To undertake the operational planning for service development. To contribute to setting and monitoring performance standards To participate in the development, monitoring, and review of departmental standards to ensure optimal client care. To maintain patient records and to collect statistics as required by the department and service. To participate in clinical supervision, in line with department protocol.
